In psm, we are alloted families. We have to visit our families and and collect data by asking them questions and note it. So here is given the ideal format of family presentation in psm.

Family composition

You have to collect data of every member of the family and fill in this table

• Total income - ___________
• Per capita income = total income
                                        Family size

• Dependency ratio = __________
It is the ratio between no. of family members having age < 15 years and > 65 years to that of between 15 - 65 years.

• Demographic family size = _______
It is the total no. of children a women can produce in its reproductive period.
= Females in age 15 - 65 yrs. + No. of children

• Family size = total no. of members

• Family type = _______ ( Joint/ nuclear/ 3 generations )

• Dependency status = ________
It is no. of children < 15 years + no. Of geriatric > 65 years.

Socio economic status -

Used for urban people

Calculate the total family score by adding the 3 scores and check the socio economic status of your family by this score card.

You have to prepare spot map of your family with proper landmarks in this way 

Its significance is that one can approach the house easily with help of it and also it is helpful for epidemiological purposes.

3. Psycho social environment

1. Relationship within family ( good )

2. Relationship with neighbours ( good )

( Ask from neighbours that how is the behaviour of this family )

3. Social problems in family ( present / absent ) 

( Like poverty, unemployment, divorce, child labour, birth of handicapped or diseased child, any disorder, etc. )

Socio environment diagnosis

You have to present your socio environment diagnosis like this -

A 6 membered joint family resides in Pucca house with adequate water supply and inadequate ventelation and lighting and having cattle shed with potential space for mosquito and flies.
